Welcome to the Deer Valley High School Counseling Website. This site should provide you with information about the counselors and some of the services they provide. The alphabet in ( ) represents the students each counselor is assigned to. You may contact your student's counselor by phone or email. If you receive voicemail, please leave a message and that person will call you back at their earliest convenience.
For registration, you will need the following:
1. Proof of Residence in the form of an APS bill or hookup statement, rental/lease agreement or home purchase statement.
2. Withdrawal Form from previous school.
3. Unofficial Transcript from previous school grades 9-12, or proof of promotion if coming from 8th grade.
4. Immunization Records.
5. Student's Birth Certificate.
6. AIMS Test Results for grades 10 through 12 if coming from an Arizona school. Test scores such as Stanford 9, latest report cards, etc. make it easier to properly place your student in classes he/she is more likely to succeed.
If you are residing with someone in their home you will need:
1. Notarized letter from the person you are residing with that you (show name)are residing in their home (show address).
2. The APS bill in the name of the person you are residing with.
If student is not living with natural mother and natural father, you will need to provide legal documentation of parent that has custody.
